BPJS Kesehatan 2027 Rates and Cap

5% combined: employee 1% + employer 4%. Cap on salary base Rp 12,000,000 — above that, premium does not grow. Family coverage included: self + spouse + 3 biological/adopted/stepchildren. BPJS Coverage Tiers — Class I, II, III

Three patient classes (room category, not premium): Class I (private room), Class II (3-bed room), Class III (multi-bed). Premium identical for all classes when employed — class determined by employee selection. Self-pay (PBPU): Class I Rp 150k/month, Class II Rp 100k, Class III Rp 35k (subsidized). Coverage Network and Referral System

BPJS uses tiered referral: start at primary care (FKTP — puskesmas, klinik) for non-emergency. Specialist or hospital visit requires referral from FKTP, except emergency. 23,000+ FKTP and 2,400+ hospitals nationwide. BPJS vs Private Insurance — Combine

Many employers add private insurance (Allianz, Prudential, Mandiri Inhealth, etc.) on top of BPJS. Private covers gaps: private rooms, specialist access, dental advanced, optical, faster service, overseas coverage. BPJS pays first, private covers balance under coordination-of-benefits (CoB). Average private premium Rp 1-5M/year for individual.
