Domestic US, UK, or Canadian health insurance does NOT cover you living abroad as an expat. International Private Medical Insurance (IPMI) — Cigna Global, IMG, Allianz Care, GeoBlue — provides true global coverage. This 2027 calculator estimates annual premium by age, coverage area, and benefits.

Why Expats Need IPMI

Your US Marketplace plan stops covering you outside the country (limited emergency-only coverage). UK NHS doesn't cover residents abroad. Canadian provincial plans suspend after 6-7 months out of country. Travel insurance only covers short trips. International Private Medical Insurance (IPMI) is purpose-built for people living outside their home country for 6+ months/year — annual policies, global provider networks, direct billing to hospitals worldwide, medical evacuation included.

Coverage Areas And The US Loading

Most IPMI plans offer 3 coverage zones: Worldwide (including US/Caribbean), Worldwide excluding US, Worldwide excluding US/China/HK/Singapore. The US loading is dramatic — adding US coverage roughly doubles premium because US healthcare costs 2-4x what European/Asian care costs for identical procedures. Strategy: buy excluding-US, then buy a short-term US plan when visiting home (saves 30-50%).

What's Different About IPMI Underwriting

IPMI is medically underwritten at enrollment — pre-existing conditions may be excluded, surcharged with a higher premium, or waived (case-by-case). Disclose fully on the application; misrepresentation voids coverage at the worst possible moment. Most plans have continuous coverage — pre-existing exclusions don't drop off after a waiting period (unlike ACA). Buy when healthy, lock in coverage early.

Expat Medical Insurance Pitfalls

(1) Assuming US Marketplace plans cover you abroad — they don't, beyond emergency stabilization. (2) Skipping medical evacuation in remote countries — air evacuation from Nepal, Tanzania, or rural SE Asia can cost $50K-$250K. Verify it's included or buy a standalone evac membership (MedjetAssist, Global Rescue). (3) Choosing direct billing over reimbursement blindly — direct-bill networks are smaller; in many countries you'll pay out-of-pocket and submit anyway. (4) Hiding pre-existing conditions — misrepresentation voids coverage during the most expensive claim of your life. Disclose fully; pay any surcharge.
