What Is Medigap Plan G?

Medigap Plan G is a federally standardized Medicare Supplement insurance plan that covers nearly all Medicare-approved out-of-pocket costs. Plan G covers the Medicare Part A coinsurance and hospital costs for an additional 365 days after Medicare benefits end, Part B coinsurance (20% of doctor visits and outpatient services), the first 3 pints of blood, hospice care coinsurance, skilled nursing facility coinsurance, Part A deductible, foreign travel emergency, and Part B excess charges. Plan G does NOT cover the Part B deductible ($257 in 2026) — the only difference from the now-discontinued Plan F (source: medicare.gov).

Plan G Premium Rating Methods

Three rating methods determine how your premium changes over time. Attained-age rating (used by most carriers) starts low at 65 and increases each year as you age — typical 4% to 6% annual increase plus inflation. Issue-age rating locks your rate based on the age you first enrolled — premiums still rise with inflation but not with your age. Community rating gives everyone in the same state the same premium regardless of age. Eight states require community rating: Arkansas, Connecticut, Maine, Massachusetts, Minnesota, New York, Vermont, and Washington (some with modifications). Community-rated premiums are higher at younger ages but more stable long-term.

Plan G Average Premiums by State (2026)

Average Plan G premiums for a 65-year-old non-smoker range from approximately $115/month in low-cost states (Texas, Arizona, Florida) to $200/month in average states (most of the country) to $290/month in high-cost states (New York, Connecticut, Massachusetts, parts of California). Female non-smokers typically pay 5-10% less than male smokers of the same age. By age 80, attained-age premiums often double the age-65 starting price. Always compare at least 5 carriers — Plan G is federally standardized so coverage is identical across carriers, only price differs (source: NAIC 2025 Medigap Premium Survey).

Plan G vs Plan N vs Medicare Advantage

Plan G has higher premiums but minimal out-of-pocket costs — best for people who use Medicare often. Plan N has lower premiums but you pay a $20 copay per doctor visit, $50 ER copay, and Part B excess charges yourself — saves money if you're healthy. Medicare Advantage replaces Original Medicare with a private plan, often with lower premiums but network restrictions and prior authorizations.
